Co-op Registration Forms

How to Register

For the upcoming 2026–2027 school year, registrations will be processed in person on Monday, June 8, 2026.

All paperwork, clearances, and payment method (checks or cash) are required at registration in order to enroll your students. Please double-check that you have all required documents and payments before arriving.
*Wait to fill out checks until you are at the payment portion of registration.

Except for nursing babies, children and teens may not attend registration, so please make childcare arrangements.

Registration Schedule

Families should register during their designated time slot:

  • Teachers: 9:00 – 11:00 AM

  • Current Families: 12:00 – 1:30 PM

  • Previously Affiliated Families*: 2:00 – 3:00 PM

  • New Families: 3:30 – 4:30 PM

If you cannot attend in person, you may send your completed registration with another person. However, registrations submitted this way will be processed at the end of your designated slot and only if all paperwork, clearances, and payment are complete.

*Criteria for “Previously Affiliated Families” will be posted once they have been finalized.


Registration Forms

Please print, complete, and bring the following forms to registration (or mail them if registering by mail):

  • Parent Contract

  • Student Contract

  • Family Registration & Medical Release Form 

  • Student Registration Form—one per child enrolled in classes


Payments

We recommend waiting to write checks at registration in case class selections change.

  • Write one $50 check payable to CHEF Co-op for administrative fees.

  • Write separate checks payable to the teacher(s) for each class’s combined tuition and materials fees.

  • If multiple children are in the same class, one combined check is preferred.

  • For classes with multiple teachers, make the check payable to the first teacher listed on the prospectus.

Teacher payments will be distributed in late June–mid July, so please ensure funds are available.

Clearances & Background Checks (Parents & Teachers)

To comply with Pennsylvania’s child safety laws, parents attending co-op must submit current clearances.

If you submitted clearances last year, you may confirm with a steering committee member during co-op days whether your records remain valid. Please check only on scheduled co-op days due to limited file access.

Registrations will be processed only when all required clearances and the mandatory reporter training certificate are complete.
Documents must be dated June 1, 2022, or later to cover the upcoming school year.
Processing can take up to two months, so please plan ahead.

Drop/Add and Late Registrations

Due to high enrollment, there will be no Drop/Add Day this year.

Auditing Policy

To maintain a productive and accountable learning environment, auditing is not permitted at co-op.
